Parent Permission for Student Website Use

There are some websites that do not come under the standards permission granted through AUP or parent permission for website use granted at registration. These different types can be found under Website Listings. In some case a teacher will send out a separate permission slip and will hold on file. In order for students to use and sign up for such a site parent permission was be granted  using the form below. This form is the kept on file at the school.
